I am unable to locate a model number as the timer has been in place for 29 years (all decals are faded or missing). I have lost the extra pins and now need 3 sets of on and off. can you help me find a supplier or can you supply ? How can I determine the model?

The 4004-20, TD-1745-20 has 40Amp contacts, 240VAC Clock and 521H Switch. The timer motor is going bad--needs lubrication often. Is a replacement module available?
