I download your repair service form but I can not type in my computer the information you request.Our church has a Verdin Carillon by Verdin and the power supply is dead. Is a +12, -!2 and =5 volts. Is made by Condor D.C. Power Suppleis INC.part # 02-31180-0001 rev G. Please advice.Thanks
