1. Red wire from capacitor. 2. Blue from motor to sw. 3. White and Green were hooked together at motor. 4. Yellow and Black were connected together at motor To reverse you changed the Red and Black The motor is wired as 220V 1 Phase. I had no wiring for the motor but I did have the Plate The switch is a square D class 2601 type Ag-1. the motor is a 1.5 hp 1 ph cont dutym 204 frame 60 hz 1725 rpm 230 v 9.6 amp code K amb 40 I thimk this is the model # 51 J17 7111 1 76D M 326
